I’m flying with my dog from NYC to Denver on United Airlines. What health certificate do I need to bring?

I already made reservations for my toy poodle to go with me in the passenger cabin. I called United Airlines to ask them what health certificates specfically I had to bring, but the person on the other line was lost and didn’t know what I was talking about. So, has anyone flown domestically (USA) with a small dog as carry-on on United? Do I need to take my dog to the vet within 10 days of departure for a health certificate? And I’m going to be in Denver for 7 days, so do I have to take him to the vet there, too, for another documentation? Thanks! Is there anything else I should know from experienced flyers?

Please suggest a good dog kennel in Denver, prefereably near Cherry Creek?

What is a good dog kennel in Denver? I would prefer it to be near the Cherry Creek neighborhood. I would like a place where my dog will be able to go outside and get exercise. Also, reasonable prices are a plus.

Where can I find a metal dog kennel in Denver?

I want a dual dog kennel for the back of my truck. I found a nice one at Bass Pro Shops, but it’s over $600.00 and I know there are other options. I could find one online, but by the time that’s over, it would be easier to buy the Bass Pro one. Does anyone know of a good place in Colorado that sells a wider selection of this type of kennel?
